Convenient, on-site personalized care.
Exceptional care begins with knowing the whole person. The Forest at Duke’s on-site Clinic offers personalized primary care with a focus on residents’ independence, quality of life, and well-being.
Located inside the community’s main building, the Medicare-certified Primary Care Clinic brings routine visits, outpatient care, and lab services close to home.
An experienced team that knows its patients
The Clinic has been associated with the Duke University Division of Geriatric Medicine since its inception. Through this relationship, Duke provides two board-certified geriatricians and a certified nurse practitioner specializing in the care of older adults. A registered nurse serving as Clinic Manager and a Clinic Administrative Manager provide additional support.
Low patient-to-provider ratios give providers time to know their patients, discuss health concerns, and develop personalized wellness plans. Care takes into consideration each resident’s needs, with attention to family caregiver support. Physicians are also available 24/7 for after-hours concerns.
