The Forest at Duke is a vibrant continuing care retirement community celebrating 30 years in the heart of Durham, North Carolina. In February 2023, we opened a state-of-the-art Small House health center, dedicated in 2025 as The Cohen Center for Health. With the opening of this innovative space, we are proud to offer a unique opportunity for new residents to join our community directly in Assisted Living.
Unlike traditional health centers that can feel fragmented or institutional, our Small House model of care is designed to feel warm, personal, and truly like home. Residents receive individualized support in a close-knit setting, guided by a collaborative care team that prioritizes dignity, autonomy, and choice. Even as health needs change, residents remain empowered in their daily decisions and supported in living life on their own terms.
Assisted Living residents receive support with daily activities such as bathing, dressing, housekeeping, medication management, and laundry. Our approach is never one-size-fits-all. Instead, we provide just the right level of support for each individual, helping residents maintain their independence while feeling safe, supported, and at home.

Assisted Living with The Forest at Duke includes an amortized entry fee, as well as a monthly fee. Memory support is offered at no additional cost to our residents. Our sales counselors can provide you with an up-to-date rate sheet upon request.
Before scheduling a tour, our administrative team conducts a brief qualification process to ensure The Forest at Duke could be a good fit for your needs and plans. This initial step helps us guide you toward the most appropriate options for residency.
Once qualified, we invite prospective residents and their guests to tour The Cohen Center for Health with their dedicated sales counselor. We can answer questions, tour the private residence and amenity spaces, and talk through next steps.
